HighPoint Hospice

Admission Criteria

Admission to the HighPoint Hospice program is made upon recommendation of your physician, based upon your needs. Normally, appropriate candidates for hospice are patients:

  • With a life expectancy of 6 months or less if the illness runs its normal course;
  • Who desire palliative treatment;
  • Who want to stay at home as long as possible; and
  • Who have a primary care person

On admission, our nurse will visit you or your family to discuss Hospice services, assess your immediate needs and recommend a plan of care.

Referrals for Hospice
Most referrals are made by a physician or a hospital discharge planner. Family members and patients can also make referrals.
